Services at LCBHC are paid for by public health insurance plans, private health insurance plans, and/or on a self-pay basis.

Public Health Insurance Plans: LCBHC provides behavioral health services to those enrolled with the Arizona Health Care Cost Containment System (AHCCCS).

Private Health Insurance Plans: For those who have private insurance, LCBHC will work with your insurance company to determine your eligibility for behavioral health services. However, due to private insurance requirements, LCBHC has a limited number of private insurance providers and thereby a limited number of private insurance appointments. Please contact us for current availability. We apologize in advance if we are not able to meet your current private insurance needs. Self-pay: For those who are not covered by either a public or private health insurance plan, LCBHC utilizes a self-pay sliding scale based on household size and household income to determine service fees for assessments, counseling services, and/or group services. Self-pay psychiatric services are also available at set rates.

We have trained staff to assist you in determining your eligibility and to enroll in AHCCCS or a federal health plan. Individuals, their family members, or healthcare advocates can contact LCBHC to make an eligibility appointment or to complete an eligibility application on the phone.
